Chronic care management is a service available to patients with Medicare coverage who have two or more chronic conditions. Medicare beneficiaries enrolled in the program receive individualized care management services between office visits to ensure they are staying on track with maintaining their health.

What Managing Health for Patients Looks Like
Our Chronic Care Management (CCM) program provides patients with additional support from their primary care provider and team.
Our CCM Program includes:

Coordinating visits with doctors and follow-up appointments

Regular phone calls from our nurse care managers to monitor health and to answer any questions from the patient

Personalized comprehensive health care plan to set goals for improvement

Preventative health care screenings

Medication education
-
On-call provider access 24/7
Medicare Cost for CCM Program Services
CCRH will bill Medicare for chronic care management services during any month that we provided at least 20 minutes of non-face-to-face care for you.
Although Medicare provides coverage for these services, Medicare beneficiaries are responsible for the coinsurance payment when services are provided. If you have a Medicare supplement, this may help cover the cost of the coinsurance payment. Please check with your Medicare supplement to learn more about your insurance coverage for this service.
Patients may discontinue their services at any time for any reason. If doing so, please notify a CCRH staff member to dis-enroll from the program.
